Yes, if you look at it in terms of the HSE's approach, which is clearly about prosecutions and enforcement, it's all about management failure.
Management failure can be either in terms of the actual management system but actually, once you start to move on from that and you look at the individual you can identify that sometimes it may be the indivudal for whatever reason, has done something they should or shouldn't have done, or maybe have been distracted.
You can have a management system that's 10 out of 10 and does what everyone expects it to deliver, it's quite effective but we can't just rely on management systems in terms of creating a safer workplace and safer community.
