I agree with the previous post. A piece of paper has never saved anybody's life.
We have a thing called Step Change which is an item called Personal Responsibility for Safety which empowers any individual to stop a job. Anyone can stop a job if they deem it unsafe. Even if there is an expert on that job, if they are not going to stop it and you explain it. No one will be worse off if you actually do that.
