Due to recent fatalities on construction sites with excavators using quick hitches for attaching their buckets the HSE has issued guidelines for recommended enforcement action.
No SSW ( management are not aware whether safety pins are required, or do not carry out routine checks)
-Consider Improvement Notice-
No SSW ( Ground workers are required to work below excavator buckets fitted with QH's)
-Prohibition Notice and possible prosecution-
No Pin fitted when it should be.
- Prohibition Notice, if more than one instance on the same site consider prosecution-
Evidence that the operator does not know how to use hitch.
-Prohibition Notice on the hitch-
Evidence of poor mainenance.
-Improvement Notice or Prohibition Notice if the hitch appeares likely to cause detachment-
No risk assessment covering bucket detachment.
-Improvement Notice-
this raises issues of proper training for the operators, as QH training is not part of the usual accepted training of these digger operators. as well as looking at the required risk assessments, method statements and maitenance records.
