Poorly secured guard-rails led to fatal fall

Two firms and two individuals have been fined nearly £200,000 over the death of a worker who fell 8.5 metres from a racking system.

Hans Zdolsek was killed in February 2004 at Wilkinson's Distribution Centre in Worksop, Nottinghamshire.
