Barkston Plastics Forming of Bolton has been fined following the death of an employee who became trapped in a vacuum forming machine which started up during maintenance.

The HSE found that the company had a written safe system of work for the maintenance, but it was routinely not followed, and the machine was not isolated from a power source. The company had not provided suitable training for managers and employees, and had no effective supervisory and monitoring system.

Trafford magistrates fined the company £20,000 for failing to ensure the safety of Phillip Ashcroft.
